Welcome to St Croix Therapy
We offer a wide array of services including Occupational Therapy, Physical Therapy, Speech Therapy, and other innovative therapy techniques. Please see below for our five steps to begin the therapy intake process. Please note that frequently we have a waiting list for appointments after 3 pm.
Step 1 - Submit Your Referral
To begin therapy services or to return to therapy we need an order from your primary care provider. These can be sent via fax to 715-386-6119 or uploaded below.
Step 2 - Complete Forms
Once you have submitted your referral, please fill out the appropriate intake forms below.
Step 3 - Verification of Benefits
Once you have submitted ALL the necessary forms we will begin the verification process. This is done on a first come, first serve basis by our Intake Department once all the required paperwork is received.
Step 4 - Evaluation Scheduling
After we have the required paperwork and verified insurance, we will contact you to schedule the evaluation. Evaluations are 1.5 hours in length for Occupational Therapy and Speech Therapy and 1 hour in length for Physical Therapy. We do require a parent or guardian to be present during the evaluation. We do not allow additional children or family members in the evaluation.
The therapist completes the report following the evaluation which will include a recommendation for services. The report can take up to 2 weeks to complete.
Step 5 - Scheduling
If the evaluating therapist recommends therapy, the Scheduling Coordinator will reach out to schedule on going therapy sessions. Therapy sessions are typically, 1-2 times a week.
